I just bought the same drive and did some testing using Indiana Jones and the Raiders of the Lost Ark.
With HL-DT-ST-BD-RE_WH16NS60-1.02-NM00100-211810291936.bin i get up around 15 read errors and the copy process is terminated.
With HL-DT-ST-BD-RE_BH16NS40-NS50-1.03-NM00000-211711201943.bin it works. I cross checked just to be sure:
1) Flash HL-DT-ST-BD-RE_BH16NS40-NS50-1.03-NM00000-211711201943.bin , get perfect copy, no errors.
2) Flash HL-DT-ST-BD-RE_WH16NS60-1.02-NM00100-211810291936.bin, read error- no copy
3) Retry to copy - fail, read error.
4) Flash HL-DT-ST-BD-RE_BH16NS40-NS50-1.03-NM00000-211711201943.bin , get perfect copy again, no errors.
Before the try out, i cleaned the disk and after flashing the pc is is rebooted.
cpu is ryzen 5950x, 32gig ram 3600mhz and msi meg x570S ace max mb.
Im pretty sure my test is accurate and that HL-DT-ST-BD-RE_WH16NS60-1.02-NM00100-211810291936.bin is bad for BH16NS40 with 1.05 firmware stock.
Other than that , MakeMKV and the tools u guys provide is absolutely gold, amazing work.
Thank you very much. Such hints are always appreciated.

However, in this case, I couldn't notice any change in behavior. I have a BH16NS40 and had installed HL-DT-ST-BD-RE_WH16NS60-1.02-NM00100-211810291936.bin from the beginning on. Recently I have bought some more BRs and now wanted to backup them (normal Blu-Rays, not 4K).

One of them even is not recognized by the drive. The drive chews on it for two minutes or so and then reports that no disc is inserted. Another of the discs can't be copied by MakeMKV because there is a lot of SCSI errors quite at the beginning of the process. The rest of the discs behaves as expected.

To solve the problem, I have followed your suggestion and have installed HL-DT-ST-BD-RE_BH16NS40-NS50-1.03-NM00000-211711201943.bin. But this did not change anything. So I have installed the previous firmware again.

Probably those two discs are really defective, although I'm asking myself how this can be the case. I'll return them to the seller.

Firmware is not the issue. Clean the discs and try again. Possibly defective. LG internal drives are pretty crappy these days. QC went downhill around 2020.
Cheers :D

You can also try to clean the laser lenses.
Do it with q-tip and alcohol (Isopropanol). Clean twice and dry immediately also with q-tip or a lint-free cloth.

Generally the WH16NS60 1.02 firmware works best for 5,25" LG-type drives, especially if it's a 3-layer disc (>66GB).

Always wash UHD discs before ripping!

Laser lens cleaning might also help.

If all still fails get a Pioneer drive as long as they are available. Preferrably with firmware before Dec. 2022 (see seller section).

@Coopervid @zittrig @MartyMcNuts

Thanks a lot! I didn't expect such fast answers given that the thread was quite old and dead for a while.

I'll follow your advice and try to clean the laser lens. I wouldn't have thought that this is important, but it sounds logical. I guess cleaning the discs wouldn't effect anything, because I have carefully examined them using a strong light source and a magnifier and didn't notice something suspicious.

I am ready to buy a new drive from whatever manufacturer is recommended :-) That would probably be a good idea anyway. I'll look into the seller section and see if somebody is willing to ship the preferred model to Europe.

I should soon (during this week) get a replacement for one of the defective discs (the one that the drive recognizes but then produces errors with). In every case, I'll try that replacement disc with the current drive. Since there seems to be interest for this subject, I'll report back.

I now have got the first of the replacement discs (the one that the drive recognized, but could not read). It was indeed the disc that was defective. The drive could read the replacement disc without any issue.

Unfortunately, the other disc will take longer to be replaced. Again, I'll report back when it has happened and when I have tested it.

Since no drive built prior to 2015 could POSSIBLY read a UHD, a drive from 2012 is "right out".

A different, newer drive is required.
No LG drive. Pioneers can.
