When I play this from the file folder using PowerDVD, it plays 00210.mpls! For science, I rented it from iTunes to compare just to verify, and .00964 is correct.

That's probably because MakeMKV names the files numerically in sequence, and the number of possible titles available for ripping will depend on your MakeMKV settings, namely the minimum duration setting. Also, I think MakeMKV is detecting it as a primary playlist in your case and placing it near the top.

Anyone know if the rental is same as retail for mpls?

I have the Netflix disc. I was able to match the segment map from what others are saying is correct on the retail disc. I'm unsure if that means I found the correct one to extract.

Is there a way to automatically de-select all the playlists? There must be 50 on this Lady Bird BD.

Nothing automatic, but right-click on any title & select 'Unselect all' from the menu.
