No need to resurrect a long-passed topic like this. The key thing is this line:
Error 'Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR' occurred while reading '/BDMV/STREAM/00066.m2ts' at offset '20759248896'
Your drive says there is a problem with the disk. Extending the retries causes MakeMKV to request it to try more, but, if the drive fails after the extra tries, there isn't a whole lot MakeMKV can do about it.
Does the disk LOOK clean, or you cleaned it? I clean any disk that gives errors like this with furniture wax, which will fill in tiny scratches that are difficult to see. I also have multiple drives available (different machines), because sometimes the alignment of the laser in one will be different enough that a disk can be read without error.

When I try to Rip, or Backup and Decode it gets stuck. So I tried to backup without decrypting and I get:

After looking at the disc it looked really weird compared to normal. Took it back to the store and exchanged for a new copy which didn't have the same defect when you looked at the disc, and reads perfectly.

Had this same problem just now.
It simply says "FAILED TO SAVE AS MKV" for the episodes, even if you only select each one, individually. And it happens with the recent MAKEMKV 1.12.3 version.
Why is that?

The Departed (2006) NTSC DVD AC3 DD5.1 - Failed during the creation of a .mkv file with no reason given for error.

Failures with v1.12.3 are wide-spread.
My personal recommendation is to go back to 1.12.2
unless you need to create a UHD dump file.

Why you might need 1.12.3 installed: UHD dump file format changed at 1.12.3, and new submissions for "missing keys" need to be made with 1.12.3 or later.
