I just downloaded MakeMKV today and have not been able to get it up and running, I've got:
Apple M5 Pro
macOS Tahoe 26.5.2
MakeMKV 1.18.4
The app just bounces indefinitely
No GUI appears
Launching /Applications/MakeMKV.app/Contents/MacOS/makemkv hangs with no output
Quarantine removed
Happens on both administrator and standard accounts
I've tried everything I can think of, if I'm missing anything please let me know!

This is what fixed the problem for me:
Temporarily disconnect the DVD drive and any USB hubs.
Force quit MakeMKV.
Completely log out of your macOS session.
Log in directly using an administrator account.
Open MakeMKV from the Applications folder.
Immediately go to:
System Settings → Privacy & Security → Security → Open Anyway
Enter the administrator password when prompted.
Apparently, on macOS Tahoe 26.4 and 26.4.1, macOS could be waiting for an administrator password while the authorization window remained invisible.
Opening MakeMKV once from an administrator account solved the issue for me. After that, it also worked normally from my regular user account.
