Scsi error / Posix error - is the disk bad?

I get this with Resident Evil Extinction:

This error sounds like a problem with the drive, or does that mean the disk is bad?

Have you tried gently cleaning the disk?

Yes, but it did not show any marks or whatever - totally clean.

I cleaned again, made sure that no speck is left - but the same, at the same offset.

I also tested another disk, which was OK.

So the disk is bad ....
