I've used MakeMKV every once and a while, currently struggling with Happy Potter and the Chamber of Secrets UHD.
For some reason I can't find instructions anywhere for where to find "dump" files. But also, I'm not sure that my issue is with the dump files? When I look in C:\Users\[user]\.MakeMKV I'm not seeing a relevant file. All I see is _private_data.tar
What are my next steps?

In my limited experience, ripping 100+ 4k Blu Rays in the last month, this is caused by one of two issues. Either an issue with the disc, in which about the only thing you can do is try cleaning the disc. Or the other potential problem is a connection issue between your drive and your system. I have an external slim drive connected via USB C. When I get those SCSI and OS Errors I disconnect and reconnect my USB C cable, eject and reinsert the disc and then try again. While ripping about 115 movies this has occurred maybe 8ish times.
I have three discs that my drive simply refuses to read, even though they are in all but pristine shape. The above process resolved the problem the other times. Good luck!

If MakeMKV creates a dump file, it'll say so in the log and include the location where the dump file is.
From glancing at your log, it seems more like the optical drive doesn't like that particular disc.
Sometimes power cycling LG drives helps them out. Sometimes gently cleaning the disc can help. Sometimes using a different drive can help.

This was a brand new disk and looks flawless. I tried a different USB port without luck (and tried power cycling).
Any other ideas? Is it reasonable to think a different bluray drive would work? I was about to embark on a major disk purchasing and cataloging effort, but this is only the 5th disk I've done (recently) and I purchased it solely to catalog and watch via a server. So I'm really bummed. I have no intention of using an actual bluray player again... I don't want to sink more money into another drive unless it really might work.
