I don't think anyone has found a solution to this? I'm currently going through the same thing with MAKEMKV ripping 0 title, 1 failed. Tried making backup ISO and using all protection settings from cell-trim, walk, full and auto. Nothing works. Any ideas anyone? This is so annoying. Common makemkv devs help us.
You need to look at the details of WHY it's failing. Odds are, you are experiencing read failures which is almost certainly related to a bad disc, a bad drive, or the combo of the two not working well together. The 'fix' for read errors is to correct them - that isn't something software can do for you.
No it's not a bad disc. It is not dirty or greasy. It's absolutely 100 percent new shrink wrapped and cleaned with a microfiber wito zero scratches and scuffs. Other Blu-ray players play the entire movie perfectly fine. And on the computer the same disc movie plays in pristine perfection on VLC.

The issue is Makemkv. It doesn't create a dump for me to send to devs. The only thing it gives are error messages and debug codes. In which no one has helped with.
I'm going to be very blunt - you're wrong. Your situation is discussed a LOT, including in many areas in this thread alone.

Brand new discs right out of the package can and do have manufacturing defects. DVD players are purposefully designed to deal with defects without stopping you from watching the movie. Software requires 100% accuracy and you aren't getting it. Change the disc, change the drive, or change both to get it to work.

Here's what I did to rip a disk that was reporting the error - Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR.

Step 1: Use makeMKV's backup tool to dump the disk
Step 2: Use the backup to extract the movie by clicking open file, go to the backup folder you just created, go under the BDMV folder, and select the index.bdmv file.
Step 3: Extract with whatever options you like as per usual

I don't see this method listed as things you should try and I don't really understand why it works in this case. Everything I read suggests my disk is bad. I've read multiple people have returned their disks, only to receive another disk that does much the same thing. I've cleaned this particular disk and it's perfectly fine with no scratches, marks, blemishes etc. It was fresh from a new purchase. I literally had it report a failure to read the disk and immediately launched into the backup tool to make the rip and the rip reported no errors. Perhaps there is a difference in the mode that is used to read the disk for backup? I don't know.

Perhaps someone that is much more experienced could explain why it would fail on a normal rip but still be able to create a working backup.
Blast from the past but I just wanted to thank you for this method. This worked for me for 3 disks that would error out on directly making the MKV file.

You are 100 percent wrong. Because if make mkv can backup the disc in question and make an ISO of the disc that you allege is 100 percent defective and insinuating that I should buy another copy. You're wrong, again. That is a waste of time and money. As Makemkv was able to perfectly make the backup ISO and be fully utilized.

Except of course when trying to rip the main movie again to no avail. This is Makemkv having problem ripping the main movie because of what? No one knows. Dirty disc or defective disc is all I read on here with no real solutions. Ok fair enough. Maybe it's not makemkvs fault. Maybe it is an unknown method of protection that no one has been able to crack. Perhaps very seldom used and ignored. That's what I think it is when 0 title saved, 1 failed means. It is a copy protection error that makemkv can't decrypt.

And I got my drive from Billy so it's not a cheap drive or faulty drive that is crap. If a drive dictates what movie you can rip and what you cannot rip then the drives being sold aren't true burners. It would be false advertising with false hopes. The point being ripping disc. In my case it's a age old DVD. I haven't ran into problems with Blu-rays yet..

@jbrip, It seems like you got MakeMKV to successfully make a backup .iso file of your DVD? But then MakeMKV stumbles when trying to open that .iso file? And it does a similar thing if it is trying to read the disc directly?

In my experience, DVDs are the wild west of physical media. I think publishers have done a number of crazy things with the file structure of the disc in an attempt to make them hard to rip. The blu-ray specification seems to be more well defined and I rarely have difficulty with blu-rays in MakeMKV.

Short of Mike asking for details about your particular disc, they only suggestion I can offer is to try different versions of MakeMKV. If you have a paid-for license to MakeMKV, you can use old versions. https://makemkv.com/download/old/ I know Mike is continuously working on handling weird DVD edge cases and the most recent MakeMKV should be the best at DVDs… However, the DVD bits of MakeMKV were rewritten in the 1.17.x version of MakeMKV. As a first step, try going back to 1.16.7 and see if it handles your disc any better.
