error- should I be worried?

Error 'Scsi error - HARDWARE ERROR:LOGICAL UNIT HAS NOT SELF-CONFIGURED YET'
I am having issues with the Queen Rocks Montreal Disc 2. Is it the disc or the drive? I got this error on my second drive while the rip failed on two drives.
and I see circular patterns on the edges of the disc after I try to rip that weren't there before the rip.

IIRC this particular disk is notoriously bad. I suspect you would need a Pioneer drive to rip it successfully.

Thanks. I cleaned the disk There are still some circular marks on it. I got this:
Hash check failed for file 00002.m2ts at offset 2059069440, file is corrupt.
Too many hash check errors in file 00002.m2ts.

No I don't have one. An Asus and an LG.
What do debug codes mean? Like this:
