I just received a brand new sealed box of X-Files Complete 1-11 on blu-ray. Discs are in perfect condition with no scratches or damage. I went to rip season 1, disc 1 which contains the first four episodes. Two of the episodes have failed so far (Scsi/Posix error) whilst episode 4 ripped fine.
I don't believe this is an issue with the drive as I've used the same drive in the same configuration for probably 100 1080P and 4K blu-rays at this point. Has anyone else had issues ripping the X-Files? I'll continue trying the other discs to see if I maybe got unlucky with season 1, disc 1.
You did not mention whether or not you've cleaned the disks; X-Files disks would be more than a few months old, and quite a lot of "older" "new" disks have contamination from the packaging. It doesn't take much to make them unreadable, while looking "perfect".
Any disk older than 6 months I've encountered needed cleaning. If you check one of the threads here, I have a picture of some new disks with contamination. It was a PITA to get that picture, too.

No, I have not cleaned them. I did look at them and they looked like perfect clean discs, no scratches, marks, blemishes etc. I didn't know I might need to clean them but I'll give that a try. Thanks.
I've continued working my way through my box set and I'm on season 4. Season 2 was the only season that didn't have any discs that failed.
So far the following discs have failed to rip (Season - Disc):
S1D1
S1D2
S1D4
S3D3
S4D1
The last disc S4D1 gave the following errors. Does anyone have any idea what could be the problem? All the others worked flawlessly, I've repeatedly tried to clean the failed discs with isopropyl and a microfiber cloth. All discs are unscratched.
The earlier posts mention UNCORRECTABLE ERRORs, which usually indicate that the drive was unable to successfully read a spot on the disc, which usually means the disc is dirty. These other errors are a bit more ambiguous, but probably mean the drive disappeared from the OS while it was in use. That's usually a power problem, meaning the drive is trying to use more power than it can get.
Is your drive a USB powered drive? Are both USB plugs inserted into high-powered ports?
Disc was brand new and cleaned with microfiber and isopropyl. The drive is supplied via two Thunderbolt 4 ports on a MacBook Pro, I don't think this is a power issue, I think it's an issue with the disc. I'll do 4 discs in a row, then the 5th disc will throw up the same error over and over again, then the next disc will work fine.
