Generally, the "source" is a plastic disk... Why would you want to set the output to be a read-only place?

My "source" is my NAS, I have 200+ ISO files to convert to MKV preserving their location.

Preferences->Video, you can set the output destination. As "SemiAuto", you can set the directory to be where your ISO files are stored, but leave the option to create directories for each disk's contents.
I use "Z:" as the destination, and this offers to create (and does if I tell it to rip) "Z:\disk_name", where "disk_name" comes from the contents of the rip. It can be edited if MakeMKV grabbed the wrong name.

I know that, but that's not what I need, my directory structure is more complex than that...

Then you're stuck with waiting for Mike to say what's possible. Best I can determine, the program assumes that the source is separate, and not something you can jump around much to, so the output is more tied down. It's possible that you can do something with the command level (rather than the menu) though - which would make sense for batch processing.
