I encountered a couple of issues when trying to rip this BD.

On Linux, it basically either failed to load the disc entirely or the drive constantly spun up and down which is caused by the following errors

Power-on or device reset occurred
Sense Key : Hardware Error [current]
Add. Sense: Logical unit has not self-configured yet
CDB: Read(10) 28 00 02 df 4c 1e 00 00 02 00
On Windows, I got farther (it was recognised) but I then failed to rip it due to key errors (that's why I submitted the dump via e-mail (svq@makemkv.com))
Have you tried downloading the latest KEYDB.cfg file, copying it into MakeMVK's data directory (i.e. where MakeMKV also writes its TGZ dump files) and restarting MakeMKV?
I also tried using a local and up-to-date KEYDB.cfg file, but that basically resulted in the same behaviour described above.
Failing that, could you upload this TGZ file into (say) OneDrive or Google Drive please, and then reply with a public download link? This would help people add your disk to the public KEYDB.cfg file.
Sure, I can do that, if sending it via e-mail is not enough. Please let me know.

Sending the email will allow MakeMKV to add your disk to its private "hashed keys" database, although this will take an unknown and unknowable amount of time - anything from days to weeks. Adding your disk to the public KEYDB.cfg file is often much quicker than this, but would require you to upload your TGZ dump file into a file host such as Google Drive or OneDrive.

Ideally, you would add your disk to both of these databases, of course ... ;).

MakeMKV must be able to disable bus encryption before it can decrypt your disk. Does MakeMKV's "Info" window say that your UHD drive supports LibreDrive? These lines from your log.txt file suggests LibreDrive should work:

Oh, it has LibreDrive enabled. I have been using this (and a different) drive for multiple years at this point to copy and decrypt BDs.
Does MakeMKV's "Info" window say that your UHD drive supports LibreDrive?
Yup, it does. This is the full output:

Sure, I can try that. But at this point I feel like the disk is bad. I tried ripping it on Linux and Windows and using two different drives. I even popped in a different UHD disk just to make sure the drives are working just fine - which they did.

MakeMKV has a transient problem where sometimes LibreDrive just doesn't enable correctly. I see nothing here to suggest there is any problem with your disk. Your best strategy is to try clearing the transient problem by power-cycling your UHD drive.

I still cannot rip it even after trying all of the following:
  • Trying to rip on both Linux (Arch-based) and Windows (10)
  • Rebooting my PC multiple times (on both OSs)
  • Power-cycling/re-plugging both of my drives
I will continue to try ripping it for a little while longer, but I'm seriously considering getting a replacement.
It being a bad disk is further confirmed by me ripping a different brand new UHD BD I received today on Linux without any issues.

A small status update on this: I just received the replacement disk and I was able to rip it without any issues. The original disk still fails to rip.
