Ripping a blu-ray disk with MakeMKV the first five minutes of the audio is missing, then it's fine. When the disk itself is played the audio is fine throughout.
Any idea what could be causing this?

Would need a lot more detail to diagnose.
First thing I would do is a decrypted backup of the disc.
Assuming that completes without any issues/errors, open the backup in MakeMKV, lookup the segment number(s) from the main playlist's segment map and try to play the corresponding transport stream (.m2ts) file directly from the "BDMV/STREAMS" folder of the backup directory in VLC Media Player.
That will confirm whether the problem is with MakeMKV or the file itself.
Disc menus are just code, like a computer program, so it's possible to instruct a player to do all kinds of weird and wonderful things, like playing different tracks starting at different timecodes.
Usually MakeMKV can/will resync these tracks but who knows?
