I purchased this title brand new, still in the factory shrink-wrap, but every single file fails to save in MakeMKV (debug log attached below).

I've cleaned and retried several times, disc looks spotless but has a kind of circular pattern on the playing surface, not scuffs or scratches, but it does look weird compared to other discs.

Any ideas?

That implies that MakeMKV was unable to successfully read the disc.

You said you cleaned the disc. Do you have a different drive you can try? It has been my experience that some drives have trouble with some discs.

Unfortunately I don't have another Blu-ray PC drive to try, yet (I'm in the market for a modded UHD drive). It does seem to play okay in my LG player, although that does tend to 'gloss-over' all but the worst errors.

I just thought it strange that MakeMKV wouldn't read any track on the disc at all, usually you get errors on some tracks and not others.

I will buy another copy of the title and let you know what happens, but if it's a manufacturing error I'll just end-up with two discs I can't read!

Just to close-out this thread...

I purchased another brand new copy of this title from a different source, and the two discs have identical "Disc Information" down to the timestamps, only this second copy doesn't have the same swirly marks on it and rips perfectly in MakeMKV.

I'm starting to think that, although it was in (what appeared to be) the factory shrink-wrap, the first copy I purchased was not new at all.

Do any discs get "re-manufactured" or has some unscrupulous eBay seller found a way to put new shrink-wrap on second-hand discs?

I'm not aware of any re-manufacturing of discs. What eBay sellers do, however, encompasses the entire spectrum of things, so maybe.
