This is one disk out of a 12-disk set (P90X), and all the other disks worked fine. I cleaned/inspected the disk, anything else this could be? And it plays fine without any issues...

Please send IFO files from the disc per http://www.makemkv.com/faq/item/8/catid/6 in addition to log file.

i dont think its an issue with the disc itself, i think it's something in the way the software handles the disc.

This is a disc mastering error, workaround will be added.

Tried this same disc using the new version of MakeMKV, and got different errors. Please see attachment and let me know what these debug codes might mean.
