Given the offset into the file and the type of errors, I would say it is more likely to be a power problem on a USB-powered drive than a dirty disk.

USB-powered drives have to be plugged into a fully-powered port (like directly into the computer, or on a hub with a separate power supply) to run at full speed, and often will simply go "off line" in mid-rip, if the ripping speed gets too high. And MakeMKV can definitely get to the top end of most drives out there.

But start with wiping the disk down with a damp, soft cotton cloth. New DVDs and BDs can have a hazy layer from the chemicals in the packaging equipment, yet still look "clean". I've had a number of disks like this since the first of the year.

The drive is not hooked up via USB. It is a SATA internal and powered off the power supply. I have the same (similar) problem with another disc (see my "Captain America The First Avenger Errors" posting) and come to find out both are from the same source off eBay. I am starting to think the disc are bad (refurbished).
