I am running OS X Lion.
So I used MakeMKV on Brides Maides which was created successfully at like 34gbs. I then opened Handbrake to convert to .mp4 and it made it fine. However; when I open the file I get this error: The document “title00.mp4” could not be opened. The movie's file format isn't recognized.
This has happened with a few Blu-Rays and I can't seem to figure out why because all my other blu-rays movies work and when they are made into a .mkv they play just fine just some seem to fail after handbrake.

How big is the Handbrake file?

This is not a MakeMKV question; it belongs in MKV file processing and playback.
Also, in HandBrake, make sure "Large file size" is enabled or make sure your settings results in an output file < 4 GB.

file size is anywhere from 3.2gb - 6gb. Only select movies have issues though, not all.
