.ISO missing features, but opening the DVD works fine?

While archiving my collection, I've tried to store the .ISO itself whenever possible, but sometimes when I open it in MakeMKV, it's missing titles (deleted scenes, etc) even though I've got the minimum title length set to 0. Those titles (features) are available when I "play" that same .ISO in VLC and navigate to them in the menu, or even when I open the DVD in MakeMKV and pull the titles in that way. Any idea why this may be?

Not all features on a disk are separate files on that disk.
