I have qnap nas and using emby server. I have a frequent notification on mkv that the title_001.mkv already exist . When i go to the nas file manager ,/ movies i see may movies title A_t001 , B_t001 etc. can i rename the replacing it with the actial mmovie title name . Do i have to include the release date also . Can i remove the _t001 extension or i have to keep it.
In the emby metadata manager i already replaced the title of the actual movies. But i guess i have to change it in the nas file manager too.

I find that it's best to rename the files during the rip, but I typically am ripping disks with 5 or more titles to be saved, and multiple disks per series. MakeMKV tries to make sure you get different file names for each, but it will start each disk with names that end with "_t00.mkv" on up.
The first thing to do is to make sure "Expert mode" is enabled; otherwise, the menu to allow this sort of thing will be hidden.
Assigning a starting directory will help; Open the options menu, and select "Semiauto", and assign a path to use as the base for ripping. I use "F:/Ripped Video", so the suggestion for the destination will be "F:/Ripped Video/{diskname}".
Then, when you first open the disk, make sure you change the first entry to a name that is appropriate for it. MakeMKV will change the actual file names to start with what you enter, and you can tweak from there.
