Source Information question???

I am new to MakeMKV and I'm trying to understand the interface.

• I only speak/read English (yeah, lazy American)
  I want the highest quality video and audio

What settings do you folks recommend?

"Settings" would be standard - MakeMKV generally extracts the audio as it is on the disk, but it can convert some more complex streams to simpler ones if your playback environment is inadequate to handle the disk audio. I generally have all audio on the disk extracted, and optionally have some tracks "simplified"... but that's a separate setting.

You can tell MakeMKV to only extract certain languages, but that can cause problems on disks that don't have (in your case) English as primary audio. I simply have it extract everything, then post-process it with handbrake to tweak the video to my needs, and to prioritize English and Japanese audio tracks and subtitles. One recent disk had 5 different languages for audio and even more subtitle tracks; handbrake let me prioritize English.
