Been having frequent problems ripping and backing up HD and UHD BluRays.
I have a mid-2014 MackBookPro Retina with 2.6 Ghz I-5, 8GB Memory running the latest version of MakeMKV, always with a full charge and plugged into power with an 85 Watt adapter. Running macOS Mojave 10.14.6.
I normally RIP to my server over 1 GbE connection that tends to only go at between 10-40 MB/S, typiucally in the teens though (seems like the NIC changes to a 100 Mb/S randomly. When backing up, I do that to a NvME USB 3.0 external drive or the main (1TB Samsung NvME SSD with over 700 GBs free and installed .)
BluRay Drive is the LG WH16NS60 flashed for UHD LibreDrive, in a Vantec External Case with a dedicated power supply. I also tried a Pioneer BDR-XD05S USB 3 external.
I have been having lots of problems ripping Blu Rays, both UHD and HD lately, although the error codes at times are all the same, and other times they change each time, either location or type.
I just increased my error read count and noticed as a result that the Soloist Disk slowed to a crawl (a few kb/s) at about 6.3 GB versus the prior 7.3GB so it was in the main movie...
Sometimes rebooting and starting over helps to complete...or backing up versus ripping, but not for this disk, which I have tried to RIP or Backup about 7 times.
Errors have been POSIX, SCSI and the OS ipc error 2-3 times with this disk (but don't recall on any other disks before.) I notice this time it reporting both ipc and POSIX errors...

You may want to back off to version 1.15.1, or force MakeMKV to use a lower speed. Some combinations of disks and drives don't work with unrestricted speeds, which became the default (when possible) in 1.15.2.
