Every title is fine except this one
Okay, that's one possible cause eliminated...

Thanks for sending the log - it is a problem in MakeMKV, should be corrected in a next version. Problem is MKV-related so you should be able to do a decrypted full disc backup and play M2TS file. http://www.makemkv.com/faq/item/4

Make sure you both send in new debug logs so Mike can take another look.

This is an incorrectly masered disc. Please submit a full debug log from 1.6.3 - it has additional logging code written specifically to catch this error. Thank you.

I also am having problems with this disc. I am using 1.6.7 and was able to successfully do a full backup and even make an encoded version with Handbrake, but i'm a purest and want the main title in MKV. :P I tried to make it with the full backup and it failed about 200mb or so in, I put in the original disc and tried to go straight to MKV with the same result, i've tried turning off audio/subtitle tracks and even ripping to a different hard drive, all failed. I've attached my log.

Thanks for submitting the log from the latest version. It's a disc mastering error and workaround WILL be added in the next version.
