I too experience "Fatal Error Occurred" on rips. macOS 15.4. I'm ripping Blake's 7 Series 1 Blu-Ray, released Nov 2024 in the UK. The first disc ripped correctly, but disc 2 and disc 5 it fails near to the end of each episode. The only error reported is about audio sync, then the dreaded Fatal Error. Sorry don't have exact error, looks like the app overwrites the log file on each run.

Before I get any suggestions to clean anything, or use a different DVD drive, I have installed the beta version of MakeMKV onto a Windows laptop, and there it's working flawlessly with the exact same external DVD drive. I'm ripping disc 5 right now, getting 6X read speeds, as opposed to 4 or 5X on the Mac, and I'm not getting lots of very noisy seeks or re-reads going on.

If you want any more info, let me know. I am a complete noob to MakeMKV, I'm following a Jeff Geerling YouTube video. I do work in IT, but I have no experience of this app. I have purchased a MakeMKV license, so I can test older versions if that has any value.

You are not receiving "Fatal Error Occurred" as described in this thread. The problem described in this thread has been resolved with the current versions of MakeMKV. All you had to do was read the first page of this thread to realize that is not the problem you are having. If you work in IT, you should understand that details are important, especially when troubleshooting.

Your issues sounds specifically like a hardware/physical issue - not software. You mentioned Windows on a laptop working. Check the cables (power, USB, etc.) you are using to connect to the Mac, if they are different. And yes, still, cleaning the disc may help. Depending on the condition/cleanliness of the disc, separate/multiple attempts to read the same disc may work/fail, and not be related to the OS/software you are running.

As a result of your post I will not be using this forum again. Would've been nice to help you guys debug the problem with the macos build, but the degree of hostility on here is astounding. Take a look at yourselves.

Can you recreate the error and share your MakeMKV log file of it or copy & paste it into a post?

The 'Fatal Error Occurred' pop-up message others were getting were caused by a change in macOS 15.4. It happened on every disc every time and rendered MakeMKV unusable on the most recent versions of macOS. MakeMKV version 1.18.1 correctly adapts to the changes in macOS 15.4.

That you have had some discs work correctly and some disc fail indicates that you're having a different problem than is described in this thread.

Generally speaking, I think the community would like to help you get past whatever's happening in your case even regardless of which thread you ask your question in.

This is all "IMHO", but:

I've been using Macs since 2012. It's been my observation that Apple's annual software releases have been getting worse, sloppier and more ill-behaved with each annual release. Recently, it's gotten truly abysmal. That said, I was able to install ver 1.18.1 on a macOS 15.5 just a few days ago - although this install was on Intel hardware rather than the newer ARM processor. The only "failure" I've experienced is that I was unable to allow MakeMKV to handle decoding chores for Handbrake... Apple's "recipe" for this Just.Does.Not.Work.

With an "unsigned" app like MakeMKV, I'm a bit surprised that there have not been more "issues" with MacOS.
