And some publishers (LIONSGATE) will program directory entries that point outside the data area of the disk, amongst the hundreds of fake entries.
BD+ introduces specific error sequences throughout the BD data, which are reconstructed using a program.
But the playable titles themselves should not have read errors in them. Player software skips bad areas simply because the CD/DVD/BD media is so fragile and error sensitive; people would have rejected them out-of-hand if the players didn't.

First of all: I use MakeMKV for quite a while now. I own several hundred Blu-Rays and DVDs with various region codes. I ripped nearly all of them over the years without any issues. Until now.
Back in december 2018, I purchased the french Blu-Ray Box of the movieseries "Les Gendarmes de Saint-Tropez". It contains 4 Blu-Rays with 2 movies per disc. I tried to rip all the discs and it worked fine with all of them - except the first movie on disc 2. It even worked perfectly all right with the second movie on disc 2 and everything else on it, but I am unable to rip the first movie on the disc. I replaced the whole box 3 times, but the problem always was the same.
To find out, if there was a problem with the disc, I put it in my Xbox One X to find out - but it worked without any issues. I watched both movies on the disc. So my last idea was to try out different Blu-Ray Drives. First of all, I tried a different Computer. I tried out an old Laptop from my mother, a Sony PCG-91111M, with Win7 Home 64bit and 4GB RAM and an integrated blu-ray drive. No success. My main Blu-Ray drive was until a few weeks ago the Asus ASUS SBW-06D2X-U. I replaced it now with a Asus BW-16D1H-U PRO. But with all 3 devices, I had no success.
I use a cheap Laptop to rip my Blu-Rays. It's an Acer E3-111-C2X0. I use it with Win7 Pro 64bit at the moment, even it has only 2GB RAM, there is a SSD in it, and I am quite satisfied. As I said, I ripped a bunch of discs in 2018 alone and never had any problems. I ripped all of discs on this cheap laptop.
Here is the full message I get from MakeMKV. The Error appeared always at the same time with the discs I returned and the new one and on all drives. It is in german. Sorry for that.

So, has anyone another idea? Should I try it out on another Computer? Maybe a Desktopcomputer, not a Laptop with an internal Blu-Ray drive? The disc looks perfect, it has no scratches or anything else that looks wrong. My main computer is a Laptop as well. It is an Asus GL752V with Win10 Pro 64bit. But the internal Blu-Ray drive is not working anymore.
Oh, and yes: I read through this Forumpage before I posted it. Yet, I decided to post this here, to find out what maybe is the problem.

Your exact errors are consistent with a USB drive being spun too fast for the available USB power from your laptop. If the laptop was not plugged in while ripping, make sure it is plugged in so that full power is available on the USB. But that still might not be "enough" power for full speed.
You need to limit the read speed. Right now, the only way to do that is to write to a media that is "slower" than your maximum read rate.
Find a USB flash drive that is at least big enough to hold the entire disk. 128GB drives are inexpensive nowadays. Make sure it is formatted as either NTFS or exFAT. Make that the destination for your rip. It should limit the speed of the read, reducing the power requirements to within limits.

No success.
The external Blu-Ray Drive was plugged in at a USB 3.0 Port, the USB Stick at a USB 2.0 Port, and as there was no success, I plugged both devices to a USB 2.0 Port, but still no success. I get the same errors at the same time. There was no difference in reading speed. Here's a screenshot just a few seconds before it aborted.

I've seen the series of error messages in one other scenario; Some drives will lock into a "retry cycle" when they encounter an error, and won't communicate with the operating system while they attempt to read the disk. The operating system times out, and reports the device not available. These cases are USUALLY ones where cleaning the disk will help. And many of them have the location of the error move around, especially if the drive DOES successfully read the "bad sector" after a while.

I editet my post. The Screenshot should be visible now. Yes, i'm getting the same errors over and over. And I got the same errors with the old discs I replaced on both computers I tested with all the drives. I will test it tonight on my other computer.

Here's what I've done to finally have some success in discs that aren't read the first time: your mileage may vary of course.
I first started running into the problem with discs that couldn't be read with a LG Blu-ray drive in a Windows 10 computer. Brand-new discs, shrinkwrapped, would not mount.
I then noticed that my Blu-ray drive had a firmware version of 1.0. Searched the LG site for the model number of Blu-ray drive and saw there was a version 1.2 of the firmware. Updating the firmware allowed me to read some, but not all, of the previously unreadable discs.
I now take those discs and plug them into a desktop Macintosh running latest version of Sierra, with a USB 2.0 external Samsung Blu-ray reader. It will read and mount and copy MKVs from the unreadable discs, but sometimes it stops reading discs as well. Unplugging and replugging the drive seems to clear something out, maybe some data error from a previous attempt, and then those last few "unreadable" discs can be converted to MKV.
I'll note that the same drive, plugged in to a current MacBook Pro with USB-C (via converters) and running Mojave, does not work. Could be the OS, could be USB-C, who knows?

I was helping a friend rip his Blu-Ray collection this weekend. Setup was a MacBook Pro with an external USB LG Blu-Ray drive, with its own power connected. Everything was going real smooth, until we came to his X-Men collection (X-Men, X-Men First Class, X-Men Apocalypse). ALL 3 kept failing with the same error on multiple tries: Scsi error - NOT READY:CANNOT READ MEDIUM - INCOMPATIBLE FORMAT.
Tried the suggested remedies - cleaning gently with soap, pat drying with eyeglasses cloth, inspecting to make sure no scratches, or other foreign material present. They all look quite pristine, even with magnifying glass.
It is curious that just the 3 X-Men Blu-Rays had the identical issue, while the others did not. He says they were bought at different times, not part of a single package. Coincidence? I think not.
Needless to say, it is too late to try to return them (they were bought long ago). I am wondering if this was an intentional error that some Blu-Ray makers put in their disks to act as a sort of "copy guard". If so, is there something MakeMKV can do to skip over the bad part (just as the blu-ray players do), so at least there is a usable rip to watch? I understand MakeMKV tries to read every byte or gives up on the whole disk, but there could be a user setting that says "Skip over bad parts", with the understanding that the rip may exhibit a glitch that the user accepts by checking this box. Just a suggestion.

The "not ready" error is coming from the operating system, and pretty much indicates that the drive has gone off-line.
Given the circumstances, the probable cause is the drive is running out of power.
I'd recommend trying to slow the USB drive down, by limiting how fast MakeMKV can write. Do you have a flash drive available that is formatted with either ExFAT or HPFS, and is big enough to hold a whole BD (64GB is usually enough)? Try using that as the destination.

Thanks for the suggestion, @Woodstock. I tried that (using a slow drive for destination). It was painfully slow, but the result was the same. Well, a little bit different may be, it gave also a "Scsi error - ILLEGAL REQUEST:ILLEGAL MODE FOR THIS TRACK", then followed by the NOT READY error cited in my previous post.
I don't understand "drive running out of power". As I said, this is an external drive with a rather large wall-wart plug that should provide it with plenty of juice. Also, it was very amazing that out of 30+ rips I did for my friend, ONLY the 3 X-Men Blu-Rays failed, all the same suspicious way. All 30+ disks had been played before several times, but seemed well cared for.

Is there any possibility of an experimental feature to mimic Blu-Ray players, and skip over tiny imperfections? I truly believe the studio is putting these in on purpose, that's the only conclusion I can reach (other than the improbable conclusion that only all the X-Men tiles have somehow gone bad in the whole collection).
