BD+ discs no longer work?

I am trying to re-rip a couple of my older BD+ discs (Epic and Percy Jackson UK versions), but they fail without error messages. Is this a problem with the new version?

Do you have debug mode enabled? If so, what does the log show? (the on-disk log has more information than the info window)
