Star Trek TOS blu-ray errors - Failed to save title
Star Trek TOS Blu-ray collection 2016 version - Failed to save title 0 to file V:/Video/Star_Trek_Season_1_Disc_5/Star_Trek_Season_1_Disc_5_t00.mkv
This has occurred on every disc I have tried so far, I have cleaned the discs and tested my drive by ripping prior known good content. Right now it appears to only be these discs. Discs have no visible damage.
What should I try?
MakeMKV v1.12.3 win(x64-release) started

Reading thru problems others were experiencing with discs I decided to downgraded from 1.12.3 to 1.12.2.
The problem stopped occurring. I don't know what this means exactly, either 1.12.3 was detecting errors and throwing alerts or 1.12.3 has an issue.
I am willing to run tests, take dumps, send extracts as needed.

Same here.
Tried to do a backup of my TOS Bluray collection to my NAS so that i do not have to jiggle 200 discs all the time and risk breaking them while removing from the most stupid disc box i ever seen.
Ended up with the same error as OP. Tried on 2 different machines. Even tried to rip it from the bluray .iso image created by other software. No luck. Solution is to remove subtitles from the selection or use older version 1.12.2 that works fine. Or use DVDFab that also works fine with the set.
BTW, my Star Trek TNG bluray set was backed up just fine.
Two more thing that i find extremely annoying with Star Trek ripping:
1) The sets contains DTS-MA 7.1 sound track but it is not selected by default. You have to manually click it every time, as only legacy DTS 5.1 track is selected from the start. That is extremely annoying, given the fact, that DTS-MA is backward compatible with DTS. Any reciever that can play old DTS can also play DTS-MA as it will only take the legacy DTS core from it. For comparison - DVDFab will select the DTS-MA track as default when in .mkv passtrough mode....
2) When multiple sound tracks with the same language are detected, only one is selected by default. That is typically the Directors commentary track that is in English and you have to select it manually every time as the software thinks that there is already another english (original audio) track....
But compliments to the author - MakeMKV is a solid piece of software that i will probably buy when it is out of beta if the price will be right...

You MAY need to tweak this to your prefs, but I use:
-sel:all,+sel:audio&(eng),-sel:mvcvideo,+sel:subtitle&(eng),=100:all,-10:eng
in the 'Advanced' tab to select all english tracks by default. Give it a try.

Now it selects all the tracks except the DTS legacy core, that is already included in the DTS-MA, but would rip into separate 2nd DTS track if selected, occupying another 1GB per episode....
As for the video - all tracks are doubled - one with original 1960´s visual effects, one with remastered 2010´s effects, listed as "angle 1" and "angle 2".
Only one angle is selected by default despite the "+sel:all". Did not find a switch to select both. Tried "+sel:mvcvideo" but no luck....

Pretty sure they are being interpreted as duplicate titles. Right-click on any title (or is it the top level? Try both) & choose 'Select all'.
For audio selection, try 'havecore'.

Getting the same kind of wacky error messages trying to rip these ST:TOS disks...
Went back to the aforementioned 1.12.2 after getting the same issues as the OP on 1.12.3, but instead of working fine, I got this message over and over...
Never had an issue with this great program before...wonder what's going on with these ST disks...

After the first 3 from the set had the issue, the next 3 so far have ripped with no problems...
Looks like the first 3 disks are bad...hopefully none of the others have any issues...
Now I have to figure out how to get replacements for the bad ones....

I didn't rip angle 2(original video), I figured I was ok with the special effects, I didn't see any complaints from other fans about the new effects causing changes to the story. I watched one episode, what I saw was mainly the warp core, phasers and other light effects.
Before I discovered that rolling back to 1.12.2 fix the problem I requested a replacement from amazon.
The original shipment was just the plastic box in shrink wrap and many of the discs were loose and the trays didn't have clips for all the discs, so 5 were loose.
The replacement pack with the same copyright dates came with the thin cardboard sleeve around the plastic box and the trays all had clips.
Examining the discs on the first box I saw areas where the plastic wasn't flat and had bubbled and some areas where the metal film on the disc was not even. I should have taken photos. The replacement box sets read fine but I was getting spin up/down on some of the original set. I don't hear my reader kick down into 0.6x often. Either terrible quality control or they were repackaged rejects. Didn't realize the quality difference between the replacement and the original until I had them in hand.
I've been burned by buying on sales before, now I don't order ahead of when I plan to rip. I made a big order one black friday and didn't find out some of my discs were bad until well after the fact. Even budgeting for buying replacements I was ahead but if there was one more disc bad in another season it would have erased all the savings.
