I have a 2008 TVGuide release of Banacek season 2.
Disc two is supposed to have 3 independent titles and a 4th that is all three combined.
When I try to convert this with MakeMKV only the first two titles are shown. There are no unreadable sectors. The disc plays correctly on PowerDVD.
Any Ideas?

It looks like this disk was authored differently - you have 4 single episodes, and no "play all" file.
"Play all" can be done multiple ways on both DVD and BD - it can be a consolidated "play list" file, like you've seen on the other disks, or entries in the menu system.
And yes, the way things are authored can change within a "set". The series "Bleach", which spans 360 episodes, alternated between "everything in one file, no separate episode files", "each episode in its own file", and "both single episode and all-in-one" over its release history. Several sets switched between disks in a 2- or 3-disk set.
