Common issues that would prevent disc from being ripped?

Let's say I ripped Deepwater Horizon on one system and tried again on another system, with the same type drive and makemkw install, and it blew out with SCSI errors.

1. check for fingerprints, dust, any sort of physical damage to the disc
2. Verify available disc space
3. Restart computer
4. Pray
5. Smoke less pot while ripping discs
6. ????
