Are you saying that MakeMKV did not suggest which title was the FPL_MainPlaylist, or that its suggestion was incorrect?

yeah it did not suggest the correct .mpls at all.

If you have the Java Runtime Environment (JRE) installed, MakeMKV should put up a message about using it whenever you insert a disk with fake playlist "protection".

I have Java installed and I don't recall seeing this popup before. For "Blu-Ray + Digital Copy" version, UPC code 774212113284, I couldn't find playlist 00667.
Running MakeMKV ripping directly from the disc did not produce the right playlist (got scenes out of order, copy warning, etc.)
Here's what finally worked for me:
1) Installed Java JRE 8 u341
2) Set MakeMKV Preferences/Video/Minimum Title Length to 1100 seconds
3) Made decrypted backup of disc using MakeMKV
3) Loaded disc backup using MakeMKV
4) MakeMKV identified title with playlist 00972.mpls, 16 chapters, duration 2:17:03, as FPL_MainFeature
5) Ripped title with playlist 00972.mpls; all segments of the resulting .mkv file were in correct order
6) Segment map: 501,502,512,508,515,510,511,504,503,507,513,514,509
MakeMKV 1.17.1, Mac OS X 10.15.7, Mac Mini 2018, LG WH16NS40 fw 1.02
