I use fedora 23. I purchased makemkv several versions ago, and it's never given me any problems through several versions of fedora and makemkv. Lately I've had trouble reading some disks at all, however. After reading the posts above, I began to suspect my drive was the culprit. I was seeing scsi errors as described and sometimes cleaning the disk would fix these.
Now the error is always the same; 'failed to save file' and the scsi error, can't read some segment (number changes as to which). I replaced the disks a couple times to no avail--so today I installed a brand new LG blu-ray writer/reader. Now, the error is constant on the latest disk:
Failed to save title 0 to file /The_Man_Who_Knew_Infinity_t00.mkv
Encountered 75 errors of type 'HashCheck Error' - see /errors/hashcheck/...
0 titles saved, 1 failed

This is a brand new, out of the box, never been used drive. It's hard to accept that this is a drive problem and I've already tried replacing the disk.

Any thoughts?

after many tries with makemkv i was able to backup the disc with dvdfab....not on first try though.

I use an external BD drive to rip my BD discs. The drive is intended to be installed inside the PC, but I use a drive adapter USB3 system to avoid opening up my desktop PC.

After repeated failures with discs that looked perfrectly fine, I decided to replace the external drive with a new one.

Problem solved.

Drives become dirty, out of alignment, and have other problems as they age. This can also happen as a drive heats up and heats the disc. By replacing the drive, I was able to overcome all of the problems that I was experiencing with the old drive.

MakeMKV used to work 90% of the time for me on Blu-ray discs. Now it's 10%.

I thought maybe my Samsung drive was bad, so I bought a brand new Pioneer drive. MakeMKV still fails to read most discs with either a "HARDWARE ERROR," a "MEDIUM ERROR," or an "Input/Output error."

I have also replaced several discs with different copies and they still fail to rip.

My conclusion is that the fault lies within MakeMKV. Something happened in recent builds, perhaps algorithmic or perhaps the read speed. Who knows?

But I can't continue to support or use MakeMKV until this is resolved and the app becomes more reliable for backing up Blu-ray discs.

I respectfully disagree.
The app is very reliable. I process approximately 1-2 Blu-Ray's each week and have done so since purchasing MakeMKV 4 years ago. I had no issues with the discs you referenced and have yet to see you list any of the errors other than "backup failed", which is of no help to any of the number of knowledgable users, here, more than willing to help troubleshoot.

Just because you replaced drives and the issues are still there does not conclude that it's MakeMKV. Are they external USB drives? I cannot tell you how many posts I've read, over time, concerning read issues with external drives.

Give us more to go on........ :wink:

It doesn't really matter what kind of error shows up in the logs. MakeMKV's error-handling sucks in general.

If a disc won't rip, but it will play on a player, then the fault lies within the MakeMKV app. The app should handle these kinds of errors, by interpolation if necessary, when there are no visible defects on the disc and replacement after replacement disc yields the same failures.

I wound up sending my 2nd brand new Pioneer Blu-ray drive back to Amazon and ordered a 3rd one (a more expensive Pioneer model). We'll see how that goes, but I'm not hopeful.

I'm using Mac Sierra 10.12.6 on a 5K iMac.

The error is almost always something like:

Error 'Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR' occurred while reading '/BDMV/STREAM/00801.m2ts' at offset '241041408'

So next, instead of backing up the whole disc, I'll usually try to find the main title and just rip it. But 8 out of 10 times those errors will still happen -- often after an hour of ripping.

This only started happening with the current version of MakeMKV. Like I said above, last month it was rare to see these errors, and usually a replacement disc would fix the problem. But now, I'm trying up to 3 copies of the same movie and the failure rates are consistently high.

This tells me that it's not the discs and it's not my drive. IT'S THIS VERSION OF MAKEMKV.

Fully agree with Lovely Rita and respectfully disagree with you. It does not suck, MakeMKV does exactly what it's supposed to do which is make a perfect bit-for-bit copy. That cannot be done with a disc that has errors, damage, etc. and therefore requires the application of error-correction. FYI, think about how much data is packed onto a disc; do you really think you can "see" a defect?

You did not answer Lovely Rita's earlier question, is this an internal or external drive?

1. If the error happens at the same point each time you try to rip the disc, the error is on the disc.
2. It's rare but I've replaced discs multiple time (once as many as 5 times) until I got one that worked. If the error is in the mastering, it could be laid down on an entire die lot.
3. I've been using the current version since day 1 and and often, it is not performing worse, in any way, than previous versions.

So yes, I respect that you're experiencing a problem but I think you are aiming your frustration incorrectly.

False - A standalone player will use error correction to skip bad areas whereas MakeMKV or any ripper for that matter requires no errors be present in order to rip a disc. Therefore, don't discount a disc may be damaged if you don't see something on the disc or it plays back fine.
I'm using Mac Sierra 10.12.6 on a 5K iMac.
I also use Mac Sierra 10.12.6 but on a homebuilt (Hackintosh) unit with a internal Pioneer drive.
Based on your assumptions that MakeMKV is the culprit why are there not more users posting with the same issue that you're having?
The error is almost always something like:

Error 'Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR' occurred while reading '/BDMV/STREAM/00801.m2ts' at offset '241041408'
Post the complete log. There possibly could be other errors listed that could help determine the cause.
This error shows that the drive could not read what it needed to at 241041408 and if that error is repeated multiple times it points to a bad disc.

Have I had that error before? - Yes!
Have I ever had to return (exchange) a disc? - Yes!
But over the 4-5 years I've been using MakeMKV it's only happened 3-4 times.

This only started happening with the current version of MakeMKV. Like I said above, last month it was rare to see these errors, and usually a replacement disc would fix the problem.
Last month?
The current version 1.10.6 was released on May 25th and you joined the forum on July 30th.
Did you come into this using a previous version, 1.10.5? When did you update to 1.10.6? Approximately May 25th or "last month" (July)?
Have you tried going back to a previous version?

So if these rip errors are always the fault of the optical drive or the disc itself, then please help me explain my situation:

I have makemkv v1.10.6 [darwin(x86-release)] running on my Mac Pro, Sierra 10.12.5

I have tried this with multiple DVDs - all the same result.

HOWEVER - when i insert these SAME DVDs and use makemkv on my Mac (host) - it reads ALL of them with no problem. Same version running on host and vm's.

So i'm not saying that i don't have something configured wrong inside of Ubuntu installs - but i have not figured out the problem yet. But this leads me to believe that not ALL medium read errors are related to dirty discs or optical drives that need cleaning.

Yes, i completely understand the difference between playing a dvd and ripping one - I've done a lot of ripping in my time, and from time to time i definitely have had problems with dirty discs and several optical drives.

But the evidence i'm seeing is that makemkv is giving me different results while using the same version of software, same DVDs and same optical drive.

Of course i have not ruled out that there may be an inherent problems with makemkv and the ubuntu versions i'm running - all i'm saying is that i've seen on so many forums (and even MakeMKV's website) where the ONLY answer to read errors is bad discs or bad optical drives.

I appear to have an exception, and i'm certain i'm not the only one that has ever encountered media read errors when it wasn't really media read errors.

That's a good question. i have tried setting host drive as my optical with both "passthrough" checked and not checked. I can use udevadm to grab header info from DVD, so Ubuntu is seeing the drive, but evidently doesn't have full access. will try to research why. Thanks for the reply!

"Additionally, if you feel the need to post about a rip error on the forum, please include the ENTIRE error report from the "conversion screen"; without this, it will be difficult for a forum member to provide accurate guidance."

I've also had some problems with read errors. Tried everything, but what helped the best was, and that's very strange, shaking my desktop when the reading error occured.

You can see when it stops writing the file. When it stopped, it gently shaked my dekstop and MakeMKV just continued...
All other times it failed creating an MKV (tried 2 discs, 2 blu-ray readers). So maybe this helps skip the problem. It helped me.

It also helps to open the tray when the error occurs and then close it again. MakeMKV continues were it left. Sometimes it just continues, sometimes not...

I have been trying this program mostly in vain, and all I can do is add to the voices saying they are getting errors ripping. I'm not going to try and place blame anywhere, but certainly every disc cannot be bad, so whether it's the drive, the program not liking my system, or whatever it seems I cannot do what should be a simple task (well, two simple tasks- ripping 3D Blu-rays then using another program to convert the result into something the Gear VR likes). I have tried two different drives on my Windows 10 machine and one drive successfully ripped a single movie, and the new drive I bought to replace it so far had been able to rip nothing. The original is a USB2 external bluray ripper which I read here will go offline if the ripping speed gets too high. It does not have a means to power it externally, and I supposed even a Y-cable didn't supply the required power. The new drive is an internal writer (seems that bluray writers are the standard now?)- the LG WH16NS40. Two discs I tried wouldn't even load the playlists while the others choke at some point during the ripping process. I will admit that these are from the library, but I did clean the discs before ripping and saw no visible scratches on them. I did try the suggestion someone mentioned of ejecting the disc while it's being processed then reloading it, but that just crashed Windows (both drives I tried).

Is there something I am missing? Am I supposed to set up the drive somehow perhaps? All I did was turn off the computer, pull out the internal DVD drive and put the new one in its place using the same cables. Windows recognized it no problem, and so did MakeMKV.

All I want to do is watch 3D movies on my Gear VR. Looks like I will have to try something else. Thanks for a program that others can use at least. Maybe I have just been unlucky and I will eventually find a disc that rips successfully, but this has certainly been frustrating.

Interesting this line: "Evaluation version, 30 day(s) out of 30 remaining." The program asks me every time I start it if I want to start the 30 days- is this normal behavior?

Your drive went off-line. This is consistent with an USB-powered drive that is not getting enough power to spin at full speed. When the rip speed reaches the power limit, the drive shuts down and resets.

If it is a USB drive that is powered by the USB port, make sure you are plugging into a full-powered hub (external power if it isn't in a desktop computer), especially if it came with a two-head cable. Plugging both heads into a USB hub that doesn't have full power available on all ports is not going to work.

At this time there is no way to limit MakeMKV's ripping speed, other than keeping it from being able to write faster than it can read. The only way I can think of to force it to slow down is to write the output to a slow device, like a USB flash drive that is big enough to hold the output file.

If you go with the flash drive route, it needs to be formatted in something other than FAT32, such as NTFS (if you're on a Windows or Linux system) or ExFAT (Mac or Windows system). In my testing, this limits the read speed to about 3.4x before this message starts appearing:
Program reads data faster than it can write to disk, consider upgrading your hard drive if you see many of these messages.
This should be well within the power limits for most USB-powered BD/DVD drives.
