Problems with MakeMKV

I am having trouble making a movie. I am gettin a couple of errors reading directly from the Movie Disc.

Any ideas?

It's 2016, and I'm having the same problems all of a sudden. Something has gone wrong. It worked to RIP discs only a few months ago. I've downloaded the latest MKV and the installed the kernel extension.
