I have run into this error on a couple of dvd's.
I have pasted the error logs.

is THERE A WAY TO FIX THIS?

Your drive says it can't read parts of the disk. The first two errors encountered were eventually figured out with retries, but then there was an error that retries didn't "fix".

First thing to try is cleaning the disk, even if it "looks clean and perfect and works when I play it in my DVD player". DVD and BD players will tolerate errors that shut down ripping, and not everything that disturbs a laser beam is visible to the human eye.

Sometimes the disks are actually defective - I have a brand new BD sitting here that looks perfect, has been cleaned twice, and still fails to read on two episodes out of 10. Three different drives think the disk is unformatted... I'm waiting to exchange it for a new copy.
