How do I remove the narrative on playback of blue ray movie?

If you are referring to the explanations added for the visually impaired, that is a separate audio track. It would normally not be selected, unless it was recorded with "better" specifications than the regular audio.
You just need to determine which track it is, and deselect it when ripping.
But, how do you know? Well, that's why I tell MakeMKV to rip ALL audio and subtitle tracks, then sort them out later, just before putting the video through handbrake to compress it. If you're not interested in compressing the video, you can use something like VLC to determine which tracks you want, and mkvmerge from the mkvtoolnix tools (google search finds it easily) to remove the unnecessary tracks quickly.

Maybe the difference is in selecting "2 channels" vs. "multi-channel", because I know I had to change the default selection rules to stop it. One of the distributors I buy a lot of stuff from LOVES to add 5.1 DTS commentary tracks to shows that have 2.0 stereo audio, and MakeMKV would drop the 2.0 channel with the default rule.
