Just started working with MakeMKV to rip my blu-rays and DVDs, first few days of ripping (with a new usb BD drive on a mid 2011 Mac OS 10.11.2) went okay, however since yesterday I have been getting various versions of the following error on multiple Blu Ray discs, eventually after numerous retries I would manage to get a full rip that plays back fine but it's gotten to the point where it no longer seems to work.

USB-powered drives must be plugged into a USB port that has enough power to support them. Laptops should be plugged into wall power if you're plugging directly into them and expecting to rip at full speed. If you're using a desktop machine, the drive should be plugged directly into the machine, OR a POWERED hub.
When MakeMKV gets them going at full speed, a lot of drives run out of "juice" from the USB port, and shut down or reset.

It is connected directly to the desktop with a Y cable so I assume it should be getting enough power. Could it be something to do with the drive mechanism being defected?

It is possible. When you rip the same disk multiple times, does the position where the error occur move around, or is it close to the same offset every time? If you rip the particular title where the failure occurs by itself, does the position move around?
Close to the same point (plus or minus 10-50 in the offset) implies a problem with the disk itself, rather than the drive.

It reads more and more like a hardware issue. Try swapping the two ports you're plugging the Y cable into; it should NOT make a difference, but I have seen it happen that a port becomes flaky. Only one port has to work for data; the other branch of the cable is only to get more power than a single port can deliver.
