Quincy Jones Live at Montreux 2008
I am getting block patterns in moving/ high contrast areas, sometimes across most of the screen.

When you ripped it, did you get any errors about "NO FUTs generated"?
Not much else to say without seeing a log from the rip to see if there were any errors. What you describe can come from a playback that can't keep up with the video bandwidth.

Re: Quincy Jones Live at Montreux 2008
"SOLVED": Your input on playback issues rather than ripping errors was correct. Although, issues were probably not on video bandwith. I should have tested this on multiple players. MPC-HC could not cope, but both KMPlayer and VLC played this flawlessly. Then I looked into the MPC-HC LAV Video Decoder settings and played with them. Looks like it is the HW decoder which fails. Turning it off, and everything displays nicely (no surprise that this PC is fast enough without the HW decoder).
I also tested with a trial version of another ripper, and had the same issues with the player.
