Your drive is having problems reading the disk. Rather than SAYING it can't read it, it is returning invalid data, and MakeMKV is trying to work around that.

Probably a smudge. Cleaning usually helps. If it is a brand new disk and cleaning does not help (yes, even new disks can have smudges on them from the packaging process!), return it for exchange.

Did MakeMKV actually say it failed saving the title?

There are times when MakeMKV CAN recover from corrupt data like this. If it cannot read and interpret something, it will say "failed saving ..." in the debug log. Make sure you have debug logging turned on (information in the FAQ linked in my signature), and watch for the additional messages.

MakeMKV has a setting to increase the number of times it will attempt a FAILED read (it's under Preferences->IO), but I don't know if that kicks in for times when the drive returns bad data, but not an error. Only Mike can answer that.
