Your drive is unable to read parts of the disk. Usual issue is dirt, finger prints, or scratches, although it is not unheard of to have a disk that was not manufactured correctly.

Again, signs of a scratched or dirty disk, unless you're seeing the same errors on other DVDs and BDs ripped in the drive.

Update: My disk was a netflix disk so I sent it back as damaged and they sent a new one and it worked fine. Maybe just crappy materials/process on this movie.

I received 2 bad discs back to back from Netflix. Just gave up; might try to get another one down the road. Seems when this happens, it's a good chance a bad batch is to blame.

When I had Netflix DVD service, I would occasionally send a disk back as "bad", and get the exact same disk again as a replacement. Sometimes, they'd polish it and send it back.
