MakeMKV failed:

Processing BD+ code using generic SVQ from builtin/00001.svq
Failed to process BD+ code using generic SVQ from builtin/00001.svq

Failed to process BD+ code using generic SVQ from builtin/00000.svq
BD+ dump directory is not set in preferences, BD+ dump not saved
Failed to process BD+ code, please check http://www.makemkv.com/svq/ for updated program and SVQ files
Failed to open disc

BD+ dump directory is not set in preferences, BD+ dump not saved
Set a directory for "dumps", so that they can be saved and forwarded for analysis...

Do you have a file named 00000005.svq in your SVQ directory? If not, please restart MakeMKV and try again. If it fails, download it manually http://www.makemkv.com/svq/00000005.svq

If it's from 2010 (date of this message exchange), it is most likely already incorporated into MakeMKV. And, if your have not disabled it, SVQ files are automatically downloaded if they exist for a particular disk.

Last of Mohicans: both makemkv 1.9.10 and 1.10.0 logs showed:
Processing BD+ code using generic SVQ from builtin/00000.svq

MacOS yosemite. can post text of crash report if you like. main error in progress window, repeated many times:
Error 'Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR' occurred while reading '/BDMV/STREAM/00800.m2ts' at offset '595261440'

00800.m2ts was main movie file.
v 1.10.0 crashed 2 out of 3 times, extracting no files.
v 1.9.10 extracted 6 of 9 files, but not 00800.m2ts, and did not crash.
blu-ray player 2.16.5 got two minutes into playing main movie, then froze.
may be bad disk, but no scratches seen on read side.

Do you have details from the OS X crash reporting tool? I'm sure Mike would appreciate the information. Send it to support@makemkv.com for him to see it.

The media read error is the hardware complaining about the disk, but MakeMKV shouldn't be crashing because of that.

o.k., will send zipped version of textedit file of crash report that macOS generated.
