Trying to run this through MakeMKV... I only end up with two files. One is a random episode from the disc and the other is a commercial. There are four episodes to a disc. Dont have the original run through log but this is what comes up when I scan a disc on the second season:

Bleach sets normally include one file with all episodes in it, plus one or two "extras". The layout within the file is:
Episode / Chapters
1 / 1-5
2 / 6-10
3 / 11-15
4 / 16-20
5 / 21-25
6 / 26-30
7 / 31-35
The subtitle track is everything in one, no "forced" flag. You can use MKVtoolnix to split them out, or use handbrake to recode them into separate files.
One exception is episodes 68-75, with individual episode files.
Another exception to this rule is the first disk of the just-released set 24, where they screwed up the mastering on disk 1; There, the overall file doesn't show up, and the episodes are split as 1 chapter for the titles, and 4 for everything else in the episode. I think this is something Mike can work around, but I haven't sent him the information on it yet. The second disk follows Viz Media's normal Bleach "formula".

And, if you're buying the whole Bleach set, you might want to check prices at RightStuf (just one F. Google will find them).

The one file I have though for the "episode" is only 40 some minutes long though and doesn't start at the first episode.

Which set number, and disk? I can check the results here, as it may be a disk issue or a problem with MakeMKV. With the exceptions I noted, everything has been either 4, 5, 6, or 7 episodes per file.
Unfortunately, the first 23 sets are in the BOTTOM box of the big stack. I know this because I have to put set 24 away...

The issues I was having were with Set 1 Disc 1, though I haven't checked what I got from the other discs. The log I posted was from Set 2 Disc 1.

One thing to test... go to http:://makemkv.com/download/old/ and pull down versions 1.8.14 and 1.9.0. Back at the end of October, I decided to recode ALL my Bleach episodes as MKV files, so I ripped EVERYTHING. 1.8.14 was current at that time, and it had no issues with ripping everything through episode 303. 1.9.0 was used for the next batch, and 1.9.1 has been used for the just-released set. Oh, wait - you won't be able to get 24 until tomorrow...

Reran Disc 1 Set 1 with the same drive ive been using.... It seems to be working? Comes out with a 1:30 some file with 20 some chapters which seems to match the 4 episode mark you gave me.

Seems to be working now.... I did encounter a "Error 'Scsi error - MEDIUM ERROR:L-EC UNCORRECTABLE ERROR' occurred while reading '/VIDEO_TS/VTS_01_1.VOB' at offset '703635456'"... any thoughts? Im attempting rerunning atm.

Assuming they're new sets, I'd say break out the USB drive and give that a try. Just make sure it is plugged in to something that has enough current available, either direct to the computer or to a powered hub.
