Drive cannot read the disk at at least two places within tracks you want. If the disk is new, you should get it replaced. If not, try cleaning it with something like Pledge furniture polish to wax over any small scratches.

You should also check around internet for any "which track is the real movie" for Expendables 2, in case this is like Expendables 3, where there are a bunch of fake titles to throw off ripping. Sometimes, those fake titles will take you to deliberate errors.

Thanks. I've requested a replacement. I tried ripping all of the tracks, hoping to find the 'real' one, but all of them failed. Will update after the replacement arrives.
