If it happens on a single disk, it is probably the disk with a problem, such as a scratch or fingerprint. Cleaning (and possibly waxing) the surface can help that.
If multiple disks have similar failures, I suggest cleaning the DRIVE, and retrying.

It's happening with only a few DVDs, so I guess it could be bad ones - odd thing is, that it's only a few rather new ones it happening to. I guess quality ain't what it used to be...
