When the operating system says there is no media in the drive, then says there is a hardware problem, there is not much that MakeMKV can do at that point, other than log the failures.
Well, that, and create a file of what it was able to extract about the disk's encryption and save it (path listed at the bottom of the log). This is suspect, though, considering the drive errors.

Hmmmm, that reminds me. I ripped Book Two of The Legend of Korra today and for each of the two discs the first time I inserted it in the drive and scanned it MakeMKV popped out a debug message, so with each disc I cancelled, ejected the disc, restarted MakeMKV and tried again and there wasn't a debug message in site... I did copy the debug message intending to email Mike about it but ultimately didn't think it was worth the effort.
