One Piece Collection 3 Disk 1 - Incorrect number of Chapters
I don't believe this is a severe glitch as it didn't prevent me ripping the disk and the titles had the correct number of chapters included.
When opening the disk, it presents me with separate titles for each episode. I assume this is a change in how they made the disks and not a change in MakeMKV itself from the previous Collections, which had one large title on each disk.
Anyways, it was strange because the first title was listed as having 7 chapters and the rest were listed as having 6. However after ripping, each title had 7 chapters. The first title had chapter numbers listed 1-7 and the rest were 0-6 (read from VLC). I don't believe this will cause me any issues, it was just a strange glitch. It had me worried that the rips were off or the chapters were going to be misaligned but I checked and the chapters line up (comparing the ripped copies to the disk playing the video in VLC).
I will point out something else odd though. In VLC playing the disk through the menus, I found that when I selected play all they were all presented in one big title. So again I'm not sure if this is a change in MakeMKV or how they made the disks.

Re: One Piece Collection 3 Disk 1 - Incorrect number of Chap
It's not a bug, the episode with 7 chapters had a first chapter starting at beginning of the episode (00:00:00.000), whereas the episodes with 6 chapters had their first chapters beginning somewhere within the episode on the DVD. A chapter marker right at the beginning of a video isn't really necessary on a DVD, it's not like it's hard to find.
-edit-
The 1st episode has chapters 1 - 7 taken from the DVD.
The rest of the episodes had chapters 1 - 6 taken from the DVD but the first Chapter didn't start at the beginning of the video, so chapter 0 was created to fill that gap.
If you're really worried about it you can go into the Profile xml and set the 'insertFirstChapter00IfMissing' option to False, then MakeMKV will stop creating 0th chapters but it will also mean the first chapters may start well into the files (And VLC REALLY doesn't like that).

The difference in chapter numbers per episode is actually fairly common in anime titles. It's a real pain when you are splitting a multi-episode MKV file into individual episodes, where episodes can vary between 1 and 6 chapters each, like the Ranma 1/2 DVDs and BDs.

Re: One Piece Collection 3 Disk 1 - Incorrect number of Chap
I feel I may have been slightly unclear. The reason it's weird is because for the previous 2 collections, MakeMKV detected the episodes as one large title and it was easy to split apart into separate episodes because the episodes consistently have 7 chapters.
This collection has been detected as having separate titles for each episode. The episode that it said had 7 chapters had numbers 1-7 and the rest listed as having 6 chapters had numbers 0-6. After ripping they all obviously have 7 chapters as they are cut up into 7 sections with 6 markers each.
EDIT:
I just noticed ndjamena's edit. It's still a little strange because MakeMKV never separated one long title into separate ones before, if that's what's happened.
I'm ripping Disk 2 now but it's reversed. Has the last episode with 7 chapters and first episodes with 6 chapters. I'll provide more info after the rip.

Re: One Piece Collection 3 Disk 1 - Incorrect number of Chap
All the episodes on the disc had 7 chapters each, but I'm sure he's grateful for that intel. DVD's are made by people and don't have to follow any precise formula. I've had plenty of series' where some of the discs had each episode in individual titles and some discs had nothing but one big title. I've even had DVDs where some episodes were converted from NTSC to PAL using speed-up, while others were converted using field (interlaced frame) blending ON THE SAME DISC. It's most likely there's something on the disc that gives MakeMKV a hint that the Play All can be split, or else the episodes are listed in both the Play All AND individually... MakeMKV is constantly evolving... have you pointed MediaInfo at them? Did you notice if the Video Frame-rate was listed as Variable or Constant? Myself, I'm wondering what THAT means.

I don't have MediaInfo to do anything with it.
It turned out that the last title recognized was actually the first episode on the disk. You were probably right about the adding chapter thing. It's still a strange result.

No MediaInfo... I'm not sure I could live like that
What would you rather happen by default when MakeMKV finds a video with no Chapter Mark at the beginning?

Re: One Piece Collection 3 Disk 1 - Incorrect number of Chap
I just wish it had said they had 7 chapters since that's the way it seems like it would've worked out to be anyways. However, you are right, DVDs really need some standardization in how they are arranged. Disk 3 is back to reading as one large title with a separate title for commentary.
Also I'm sorry if I came across like this was a huge issue. I just thought it might've been a small typo in the code or something. Since I'm unfamiliar with DVD structure it just seemed strange to get such varying results.

Weird results. I've gone to watch my show now that it's ripped using XBMC. Suddenly some chapters are out of order or missing entirely. I'm not sure what's going on but it may be tied to some chapters having the same UID.
Any idea how to fix this?
P.S. I'm posting in here because I figured it was relevant since it's dealing with the same disks.
EDIT:
Also I'm re-ripping to double check if this is a problem with MakeMKV or Handbrake.
EDIT2:
I'm basically 100% certain it's Handbrake at this point. Well over to their forums for some sort of solution. Sorry for reviving a thread unnecessarily.
