Tried 2 different disks several times and get the same error's. This is the first disk I've never been able to rip. Assuming its the LG drive based on the error messages but cant find any new firmware etc. Any help would be appreciated.

disc is damaged or dirty.

Both discs are visually perfect. not sure whats going on.

If your eyes can see 50gb of 1's and 0's then you're the next level of human evolution.

The message is from your drive, MakeMKV is just passing it on to you, and the message is that your drive can't read the disc.

I appreciate the help but really? Cant even escape such comments here either?

Interesting that both disks look great (better than many others I've used) and the only title I've ever had issue with. Wonder if they're just a bad batch?

Correct. And bad batches tend to be shipped to the same store, so it may take a while before you get a good one.

Worked for me with a title I was having trouble with. Took longer to rip, but worked.

I had the same problem. Tried to rip to MKV and had read errors. Watched movie without difficulty. Tried to do backup of disc...hash check failures...'file corrupt.' Not sure this is a damaged disc. I don't know anything about the tech of ripping movies, but this 'seems' like copy protection to me.
