So, what does it take for BD+ newer titles ?
I did my homework, googled it, wikipedia'd it. For the non-savvy decrypto process in me ( savvy in other stuff ), what does it take, plain and simple for the common mortal ?
A BD+ drive ?
A newer and improved makemkv that washes cleaner and brighter ?

My understanding is that whatever version of makemkv you have, it transparently "phones home" to get the latest decryption keys available. I have never encountered a blu-ray disc that it couldn't decrypt, which is justification enough for the cost of a makemkv license. You may find discussion of problems with just released titles on this forum, but it isn't long before the necessary keys are found and the database updated for everyone - again in a transparent manner.

Oh, and there is nothing special required of the BD drive for BD+. It's the decryption software that matters. Similarly, BD drives themselves don't have regions like DVD - software must enforce regions, which makemkv chooses not to do.

As specific examples:
Enough Said 2013
All is Lost 2013
Both could not be decrypted. Protection shows "AACS v39, BD+ v2013.11" while latest available is v43. If it does matter, I've tried with MakeMKV 1.8.7 and 1.8.8 on MacOS-X Mountain Lion with the same results.

You will sometimes see a report in the log that says that the BD+ was not properly decoded, no FUTs (fix-up tables) were found. There will then be a clickable link to submit the necessary information to MakeMKV support for Mike to generate a new SVQ file for that disk.
When he does, the SVQ will normally be automatically downloaded as needed, so "suddenly" the disk will work.
Best practice is to click the link, then post a message here about the disk or disks (and the fact that you clicked the link so that we don't ask you to do it again ), and Mike will normally post an "SVQ Updated" message after he's created the new SVQ.
BD+ decoding is independent of the AACS encryption, by the way. AACS determines whether or not the drive will let you access the disk, while BD+ corrects deliberate distortion of the data on the disk.

This pops up sometimes on Macs, and not just with MakeMKV. In searching the web, there are a lot of "Oh, I rebooted and it works now!" answers when the question is asked. Is this a USB-powered drive? If so, are you using the 2-plug cable to connect it, so that it can pull more power from the USB ports?
