I have been using Make Mkv for a while now with no problems until not.
I am trying to back up a dvd film (HEAT 1995) but coming up with failed.
Log as follows.

The medium errors are because your drive cannot read parts of the disk. When that happens, the rip fails.
Standard drill is, clean the disk. My personal addition to that is cleaning using something like Pledge furniture polish, which applies a thin coat of wax while it cleans.
There are steps beyond cleaning, up to and including getting the disk replaced, but start with cleaning. It's also possible the drive is getting dirty, but that will show up with errors on multiple disks.

I get these errors also even though the disc looks fine, Problem is when they stamp the DVD after while the stamp wears out so the disc will fail quicker or right away (bought a few I had to wait a bit to exchange so a new batch is circulated). Odds are if this is the only disc to do this than it's the disc. First do Woodstock's advice! (trying the pledge trick now)
