I've tried about 5 times. Even sent the disc back to thinking it was damaged, but I'm getting errors with the replacement disc as well. Rebooted. Powered off/on external BR drive. Powered down BR drive, pulled power and firewire, and then plugged back in. All with the same result.
I've been using MakeMKV for nearly 3 years. I've never seen this error before on any DVD/BR.

I just tested a few other BRs and they worked fine. Doesn't seem to be a hardware issue.

Your drive is telling you it can't read the disc. There are only 2 options:
1. Bad disc.
2. Bad drive.
Its no surprise that you have a replacement which also has the same issue, poorly pressed discs come in batches. Just return it again and ask for another.
