I picked up a used box set of all of the Star Trek TNG episodes on blu-ray (2020 CBS Studio). I've only got one old PC that has an (old) blu-ray drive so I wanted to use MakeMKV (v1.18.2, on Windows) to rip the discs to watch on another PC.
Of the 41 discs, 40 went well, but I'm unable to extract a couple of the episodes from one disc. There's no obvious signs of damage on the disc.
The blu-ray drive is a Lite-On iHBS112 2, which initially had firmware CL04. I upgraded it to the latest (PL06) but it still fails the read the disc. I've attached the log files from a CL04 and a PL06 run.
I tried an alternative technique I spotted on the forum: start a backup with MakeMKV, uncheck the decrypt option, let it run until it gets to the first m2ts file, stop the backup, quit MakeMKV, delete the incomplete BDMV folder and drag-and-drop the BDMV folder from the disc to the backup folder. Eject the disc, start MakeMKV, select File>Open Files and double click the discatt.dat file created by the previously cancelled backup.
When copying the BDMV folder I had to retry a few times, but it did eventually succeed. However, the MakeMKV still couldn't get all the episodes from the copy of the blu-ray in the backup folder.
Based on the above - is it more likely to be a bad disc, or my old blu-ray drive that's the problem?

More likely a dirty disk. I've ripped the entire TNG series on BD in the past (and will be again soon), so I know it works.

I had given the disc a gentle clean after the first failed attempt, but unfortunately that didn't made a difference.
As I'm not the original purchaser of the set I guess I might be out of luck in getting that disc replaced.

Hello - I have just started ripping TNG blu-rays to add to my media server and need some help please.
Also, if your disc is damaged, I could give you an iso or something to help? I have been watching some of my older boxsets like Stargate and found some discs have gone bad (rot), guess they used the cheapest media possible
So, my issue is starting with disk one, looks like 3 episodes should be on there but I found the 3 titles I ripped do not match when I watch them back and compare to the disc and the intro is missing on all of them, what I am doing wrong
and is there any way to better automate or otherwise identify which title matches which episode?

My workflow is to make a decrypted backup of a disc (on the first screen of MakeMKV, the icon of the yellow folder with the green arrow). I open the backup in MakeMKV. Highlight the title on the left I suspect of being an episode. Take note of the number(s) in the segment map in the info box on the right. The number in the segment map correlates to a .m2ts file in the <backup>/BDMV/STREAM folder. Play that .m2ts file with your favorite player (I like MPV. VLC and others work great too.) Figure out which episode it is. Maybe verify which audio tracks are commentary tracks, etc. Update that info in MakeMKV. When all the episodes and extras are correctly identified, Make the .mkv files. It'll go really fast since it is not reading from the optical disc.
Expert Mode probably has to be enabled. I've run with expert mode on for so long I often forget it is even an option (the General tab of MakeMKV's preferences).
I use a relatively cheap external SSD as my 'scratch space' to store disc backups and process them into .mkv files. I like making the backup separately from making the .mkv files. Making a backup requires almost no effort on my part and can run in the background. This means when I buy a new disc, I can easily verify that the disc is good (by successfully completing a backup) during the return window for the disc. Then, when I'm ready to devote a few minutes of focus to it, I can make the .mkv files with a high degree of confidence that they're labeled correctly. When I'm happy with the output, I move the .mkv files to their permanent home and delete the other stuff.

Not sure why but there was 2 titles that were 1.5 hours that confused me on disk one. One of these is far point part 1 and 2 and the other is 2 random episodes that is not playable from the DVD menu, that is what caused me to lose my mind. I think I'm okay now, got to episode 14 just ripping what looks like the right titles and comparing with playing them from the DVD. just wish we had a more "automated" way of doing this - where is AI when I need it
