Error 'OS error - This device does not exist'

While ripping UHD Game of Thrones S02 Disc 2, I received this error message:

Error 'OS error - This device does not exist' occurred while reading '/BDMV/STREAM/00001.m2ts' at offset '0'

This corresponds to E05 (and also happens on E06). Episode 04 ripped with no problem.

I've tried Cleaning Disc and another drive with the same results. Anything else to try before getting a different disc?

Is the drive USB powered? It might not be getting enough power. Connect all the USB plugs to more powerful ports.
