Error 'OS error - STATUS_DEVICE_NOT_CONNECTED' occurred while reading <file> at offset <offset>'

Having an issue where MakeMKV can correctly identify all titles on a video, but cannot save them. I get the dreaded: "Copy complete. 0 titles saved, 2 failed" message.

Skip to the bottom to see my device information. Full log is attached. The title of the post is the first error that occurs, but there are others:

002003:0000 Error 'OS error - This device does not exist' occurred while reading '/VIDEO_TS/VTS_01_1.VOB' at offset '27000832'
002003:0000 Error 'OS error - The volume for a file has been externally altered such that the opened file is no longer valid' occurred while reading '\Device\CdRom0' at offset '27000832'
I know there is not an issue with the disc itself, as the video files can play just fine: Image

The drive is having trouble reading a section of the disc and is running out of juice. Clean the disc before trying again.

Playing and ripping are totally different. Players can buffer but ripping requires the drive to read every bit exactly as on the disc.
