Depends on what your disks contain. I've had BDs with 56 episodes in a single file that I had to break out. Using handbrake, I separated them into individual episodes.
Duplicated episodes usually have something that differentiates them, like language. And sometimes episodes were in non-standard order.

MakeMKV doesn't know which episode is which. It doesn't even know the difference between a TV show or a movie. It just displays the things it finds on the disc in the order it finds them. Unfortunately, it is up to the user to sort out which is which.
